How to Become a Client
*Please note: Aqualitas will be able to accept patients once we receive our sales licence from Health Canada. To be contacted when we are registering clients, please email firstname.lastname@example.org or call 1-833-300-AQUA(2782).
There are a few simple steps required to become an Aqualitas client.
- Have your physician or health care practitioner complete your medical authorization form. You can download our medical document here to bring to your medical appointment.
- Once your health care practitioner has completed the medical authorization, you will need to fill out our registration form and send both documents to Aqualitas. If you’d like help completing your registration, don’t hesitate to call our Client Care team who would be happy to walk you through the process.
- Shop for your products! Once we have received your completed medical authorization and registration form, you can log into your client account and purchase your medical cannabis. Our Client Care team is available to help you complete your purchases.
